Emergency Contact Form
Each elementary school student (TK through grade 8) needs to have an Emergency Form completed by his/her parent/guardian and kept on file at the school office. The Emergency Contact Form includes the following information:
- Parent's complete names, address, and home telephone number.
- Parent’s work telephone numbers and cell numbers.
- The name, address and telephone numbers of another adult who can be called in an emergency when a parent cannot be reached.
- Medical alert information.
