Immunization Requirements
California law requires all students enrolled in California Schools to have up-to-date records. The immunization policy each age level is as follows:

5 doses (4 doses OK if one was given on or after 4th birthday) last dose was before 2nd birthday. At least one dose must be Tdap or DTap/STP given on or after 7th birthday for all 7th -12th 1 dose of Tdap (Or DTP/DTap given on or after the 7th birthday)

2 doses (Both given on or after 1st birthday. Only one dose of mumps and rubella vaccines are required if given separately) Dose 1 dose (given on or after 1st birthday. Special Exemptions: There are some special exemptions (waivers) allowed by the State (medical and/or personal beliefs). Personal Belief Exemptions will not be granted after 12/31/2015 per state law. These exemptions will be approved when the appropriate forms are completed.
