Employee Accident investigation Form-for Supervisors

The following form is to assist supervisors in determining root cause of employee incidents, as well as identifying and correcting any hazards that may exist to prevent similar incidents from occurring. Incident investigation and documentation is a Cal OSHA requirement and is stated in the District’s Injury and Illness Prevention Program:

Transportation Deduction

As an employer with 50 or more full-time employees, the Orinda Union School District is subject to a new law that requires employers to provide a commuter benefits program to its employees who work an average of 30 hours or more per week. Senate Bill 1339, signed into law in 2012, went into effect September 30, 2014. The Pre-Tax Benefit, or Option 1 of the program, allows employees to exclude their transit or vanpool costs from taxable income, to the maximum extent permitted by federal law (currently a maximum of $130.00 per month).

Envoy Retirement Saving Plan

We would like to make our employees aware of the retirement plans that we sponsor. These plans include a 403(b) Tax Sheltered Annuity Plan (TSA/403(b) Plan) and a 457(b) Deferred Compensation Plan (DCP/457(b) Plan) (the Plans). Participation is voluntary, allowing you to make pre-tax salary deferral contributions via payroll deduction. Please see the below link for further details:

Dependent Verification Review

Our Health Plan Provider, CalPERS, is requiring that the District have an outside auditor verify the eligibility of each dependent on our District sponsored medical and/or dental plans. Therefore, starting February 3, 2014 and continuing through February 13, 2014, a mandatory Dependent Verification Review will be conducted for all benefit eligible employees (both full-time and part-time).  Please reference the following links for more details:
